Notice of Privacy Practices for Idaho Voice and Swallow Center

This Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. Idaho Voice and Swallow Center is committed to protecting the privacy of your health information in accordance with applicable laws and the Health Insurance Portability and Accountability Act (HIPAA).

Use and Disclosure of Health Information:

Idaho Voice and Swallow Center may use your health information, within the confines of HIPAA regulations, for purposes of providing you treatment, obtaining payment for your care, and conducting healthcare operations. Your health information may be used or disclosed only after Idaho Voice and Swallow Center has obtained your consent or authorization, except as required or allowed by law.

• The Right to Request Limits on Uses and Disclosures of Your Health Information

  You have the right to ask that we limit how we use and disclose your health information. We will consider your request, but we are not legally required to accept it.

• The Right to Choose How We Send Health Information

  You have the right to ask that we send information to you at an alternate address (for example, sending information to your work address rather than your home address) or by alternate means (for example, email instead of regular mail).

• The Right to See and Get Copies of Your Health Information

  In most cases, you have the right to look at or get copies of your health information that we have. There may be a small charge for the copies.

• The Right to Get a List of the Disclosures We Have Made

  You have the right to request a list of instances in which we have disclosed your health information for reasons other than treatment, payment, or healthcare operations.

• The Right to Correct or Update Your Health Information

  If you believe that there is a mistake in your health information or that a piece of important information is missing, you have the right to request that we correct the existing information or add the missing information.

Changes to This Notice:

Idaho Voice and Swallow Center reserves the right to change this notice. We reserve the right to make the revised or changed notice effective for health information we already have about you as well as any information we receive in the future.

Complaints:

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. All complaints must be submitted in writing. You will not be penalized for filing a complaint.
